Employer Faces Over $90K For Safety Violations

OSHA alleges the company violated 11 regulations and is to pay $91,832 in proposed penalties.

The Occupational Safety & Health Administration (OSHA) recently cited Supplyside USA, an Ohio pallet manufacturer, for violations related to machine safety following a serious injury to one of its employees. OSHA alleges the company violated 11 regulations and is to pay $91,832 in proposed penalties.

Supplyside USA, operating as Prime Woodcraft, Inc., allegedly had the following violations:

• Did not have energy control procedures in place (Lockout/Tagout to control unexpected energization of equipment);
• Did not have machine guards in place when workers were using machinery;
• Did not have a housekeeping policy in place to control accumulation of dust and reduce the risk of combustible dust hazards;
• Did not have an effective Hearing Conservation program;
• Was using power from a flexible power cord on a permanent basis instead of establishing a permanent power receptacle;
• Did not have a hazard communication (HAZCOM) program;
• Did not report the in-patient hospitalization of a worker to OSHA within 24 hours; and
• Did not provide injury and illness records (OSHA 300 Log and OSHA Form 300A) within four business hours.

Supplyside USA (Prime Woodcraft, Inc.) is permitted 15 business days from the receipt of its citations and proposed penalties to pay the penalty and comply with the citation’s directives, schedule an informal conference with OSHA’s Area Director, or contest the findings in front of the independent Occupational Safety & Health Review Commission.
